Escherichia coli O157:H7 is a highly virulent serotype of enterohemorrhagic E. coli (EHEC) and a major global foodborne pathogen, identified by its O157 somatic and H7 flagellar antigens. Transmitted mainly through contaminated food (like undercooked beef, unpasteurized milk, and produce), its low infectious dose necessitates robust, rapid detection. Pathogenicity centers on two factors: the production of potent Shiga toxins (Stx1 and Stx2), which damage the vascular endothelium, especially in the kidneys; and the ability to create "attaching and effacing" (A/E) lesions on the intestinal epithelium. The resulting clinical syndrome, Hemorrhagic Colitis, can progress to the life-threatening Hemolytic Uremic Syndrome (HUS), which causes acute renal failure and neurological damage. Diagnostic accuracy based on both O157 antigens and Shiga toxins is vital for patient management and epidemiological tracking.
