Inducible T cell costimulator ligand (ICOSLG), also known as B7H and CD275, is a single-pass type I membrane protein. This protein is a ligand of ICOS, a T-cell-specific cell surface receptor. The ICOS-ICOSLG pathway has important clinical significance for the immune system.

ICOS-ICOSLG Signaling Pathway

Attracting cognate Tfh cells and engaging in T-B collaboration is critical for the differentiation of GC B cells. Here we list the role of the ICOS-ICOSL pathway under conditions with or without TCR engagement in Tfh migration and T-B collaboration.

Transient and repetitive ligation of the ICOS-ICOSL without TCR engagement functions as the following (Figure 1A):

  • Induces transient actin remodeling, which involves programming and motility of T follicular helper (Tfh) cells.
  • Facilitates cognate T-B conjugation and its entry into the germinal center (GC) of lymphoid organs to generate high-affinity antibodies.

Ligation of ICOS-ICOSL with TCR engagement in the TCR and peptide-MHC complex works as follows (Figure 1B):

  • Facilitates CD40L externalization causing CD40 co-stimulation to GC-B cells.
  • Increases translation of IL-4 mRNA and accurately delivers CD40L and IL-4 for effective selection of GC-B cell clones with high affinity for the internalized cognate antigens.

Clinical Significance of ICOSLG

ICOS and ICOSLG are closely associated with various diseases, such as colorectal cancer, acute lymphoblastic leukemia, autoimmune diseases, immunodeficiency, and other inflammatory diseases. Therefore, ICOS-ICOSLG signaling is a promising biomarker and target for immunodiagnosis, prognosis, and therapy.

  • In preclinical or clinical studies, the agonistic antibodies targeting the ICOS-ICOSLG pathway could induce tumor regression through the regulation of T-cell immunity. These antagonistic antibodies have been used for cancer treatment.
  • The rationale study about this signal reveals that the ICOS-ICOSLG pathway may help improve vaccine prevention and immunotherapy treatment of immune disorder diseases and cancers.
