Neuregulin 1 (NRG1, aliases NDF, GGF, ARIA, etc.) is a type of membrane glycoprotein of the neuregulin family, encoded by the NRG1 gene located in region 8p12 on chromosome 8. NRG1 is structurally related to epidermal growth factor (EGF), possessing an EGF-like motif, which is responsible for binding and activating homodimerization or heterodimerization of ERBB molecules, the members of human tyrosine kinases of the ERBB/HER receptor group, thus triggering downstream signaling pathways involved with many biological activities.

NRG1 Functions and Signaling Pathways

NRG1 is generated in several isoforms by alternative splicing, which enables it to perform a diversity of functions, especially for the normal growth and differentiation of many organ systems, including the heart and the nervous system, the intestinal epithelium, and skeletal muscles. The dysfunction of NRG1 has been demonstrated to be involved with several diseases, including lung cancer, schizophrenia, and bipolar disorder. On the contrary, overexpression of NRG1 leads to abnormal dendritic spine growth, decreased synaptic plasticity, and disrupted excitatory-inhibitory connections.

NRG1 exerts functions through ERBB/HER receptors, as all the isoforms of NRG1 share the structural EGF-like motif and are capable of binding to homodimerization of ERBB 4 or heterodimerization of ERBB2/3, ERBB3/4, and ERBB2/4, thus activating downstream signals, including the Hippo pathway, P13K/AKT pathway, ERK/MAPK pathway, and the Wnt pathway, subsequently regulating a wide range of physiopathological processes such as cell proliferation, hypertrophic growth, apoptosis, oxidative stress, cell autophagy, cellular inflammatory response, and vascular proliferation. The NRG1-ERBB signaling axis is a key regulator of cardiac development, participating in the complicated cross-talk between the myocytes and microvascular endothelium in the adult heart.

NRG1 as A Therapeutic Target

Inspired by the various and essential functions of NRG1, the development of NRG1-based targeted therapies has attracted much attention during the past decades. Therapies targeting NRG1 fusion utilizing anti-HER2/3 antibodies and ADCs have been proposed, while there are also multiple clinical trials ongoing attempting to directly target or amplify NRG1 in different diseases and conditions, including heart failure and multiple neoplasia. Several preclinical and clinical trials at different stages are being conducted, assessing the efficacy and safety of targeting NRG1 in treatment and prognostic models, as well as serving as diagnostic tools. Besides, anti-NRG1 antibodies have also been continuously assisting in research investigating the mechanism of a wide spectrum of physiopathological processes in diseases, guiding for future development of therapeutic regimens.
