Pro-Brain Natriuretic Peptide (proBNP) is a 108-amino acid prohormone predominantly synthesized by ventricular cardiomyocytes in response to increased hemodynamic stress, particularly ventricular volume expansion and wall tension. proBNP acts as a critical signal in the body's attempt to maintain fluid balance and reduce cardiac strain. Upon release into the circulation, the prohormone is rapidly cleaved by convertases (such as furin) into two fragments in equimolar amounts: the biologically active C-terminal peptide, Brain Natriuretic Peptide (BNP-32), and the biologically inactive N-terminal fragment, NT-proBNP.
NT-proBNP has emerged as the superior clinical biomarker for heart failure due to its extended plasma half-life of approximately 60-120 minutes (compared to the short 20-minute half-life of active BNP) and its higher circulating concentrations. Its stability makes it less susceptible to pre-analytical errors and more suitable for reliable measurement in diverse laboratory settings. Elevated NT-proBNP levels are highly correlated with the severity of ventricular dysfunction and are universally accepted for the screening, diagnosis, and prognosis of acute and chronic heart failure, as summarized in widely cited literature. Measurement of NT-proBNP is foundational in understanding the natriuretic peptide signaling pathway, which operates by activating the natriuretic peptide receptor A (NPRA), leading to downstream effects of diuresis, natriuresis, and systemic vasodilation.
