RBP4 (Retinol Binding Protein 4), also known as Plasma Retinol-Binding Protein or PRBP, is a member of the lipocalin family of transport proteins. RBP4 is found to be primarily present in the liver, followed by adipose tissue depots. In addition, RBP4 mRNA is also detected in other tissues, including the kidney, testis, lung, and brain. Furthermore, the special β-barrel structure of RBP4 allows the specific accommodation of a retinol molecule, which contributes to the solubility of this hydrophobic vitamin in an aqueous environment.

RBP4 is the only carrier of retinol, a key element in the visual cycle. Therefore, RBP4 serves as a pivotal carrier for retinol transport from the liver to peripheral tissues. Furthermore, evidence reveals that reducing RBP4 with antagonists severely affects the development of vision. Therefore, RBP4 has gradually emerged as an attractive target for the therapy of eye diseases.

Abnormal RBP4 is closely linked to a variety of metabolic diseases, including fatty liver disease, metabolic syndrome, and atherosclerosis. Additionally, high levels of RBP4 may contribute to chronic diseases such as psoriasis. Research has shown that RBP4 is also related to ankylosing spondylitis and rheumatoid arthritis (RA). Notably, changes in RBP4 levels may also be associated with the development of colorectal adenomas, colorectal cancer, and breast cancer. Research reveals that RBP4 serves as a promoting factor for breast and colorectal cancer stem cells in culture and animal models.
RBP4 induces the expression of pro-inflammatory cytokines in macrophages by activating the JNK and TLR4 pathways. On the other hand, the binding of RBP4 to STRA6 recruits and activates JAK2 and STAT5, which leads to the expression of STAT-relevant molecules, such as peroxisome proliferator-activated receptor gamma (PPARγ) and suppressor of cytokine signaling 3 (SOCS3).
