CFB (Complement factor B) is a crucial component of the complement system's alternative pathway, which is part of the innate immune system, produced mainly in the liver and presenting in the blood plasma. The complement system induces inflammation, targets the pathogen's plasma membrane, and improves the capacity of antibodies and phagocytic cells to remove pathogens and damaged cells.

CFB Pathways

  • Alternative Pathway Activation: In the presence of a pathogen, CFB binds to C3b, a fragment of the complement component C3. The C3bB complex is created when CFB binds to C3b. After that, CFB in the C3bB complex is broken down by Factor D, an additional complement system component, producing the fragments Bb and Ba. The Bb fragment remains attached to C3b, forming the C3bBb complex, which acts as a C3 convertase. This enzyme complex is crucial for the amplification of the complement cascade, leading to the production of more C3b and the subsequent steps that lead to pathogen opsonization, cell lysis, and inflammation. The Ba fragment has a lesser-known role but can have regulatory functions.
  • Amplification Loop: CFB amplifies the complement cascade by stabilizing the C3 convertase and facilitating further complement system activation.

Functions of CFB

  • Opsonization: CFB aids in opsonization by encouraging complement proteins to adhere to pathogen surfaces, designating them for phagocyte destruction.
  • Inflammation: CFB participates in the inflammatory response by generating pro-inflammatory peptides upon activation.
  • Role in Immune Response: The C3bBb formed by CFB is essential for the propagation of the alternative pathway, leading to a rapid response to infection. This pathway operates independently of antibodies, providing an immediate defense mechanism against pathogens.

Clinical Relevance and Related Diseases

  • Individuals with immune system failure are more vulnerable to infections due to mutations or abnormalities in CFB.
  • Overactivity of CFB can contribute to autoimmune diseases, where the complement system might attack the body's cells.
  • CFB in aHUS: Dysregulation of the alternative complement pathway, including mutations in CFB, can lead to aHUS, a rare but serious condition characterized by microangiopathic hemolytic anemia, thrombocytopenia, and acute renal failure.
  • CFB in AMD: Hereditary differences in CFB have been linked to a higher chance of AMD, which is the primary cause of visual loss in the aged. Dysregulation of the complement system, including CFB, contributes to retinal damage and inflammation in AMD.

Applications of CFB Antibodies

  • Therapeutic Target: Antibodies targeting CFB are being investigated as potential therapeutics for complement-mediated diseases, including aHUS and AMD.
  • Research Tools: CFB antibodies are also used as research tools to study the role of complement activation in various diseases and to develop novel treatment strategies targeting the complement system.
