Dickkopf-1 (DKK1) is a suppressor of the Wnt signaling pathway and is one of the members of the Dickkopf family. Among four main secreted glycoproteins of the Dickkopf family, DKK1 is one of the most extensively studied members, playing pivotal roles in various cellular behaviors such as proliferation, migration, adhesion, and polarity, and it exerts critical effects on the progression and prognosis of various cancers.
Comprising 266 amino acids, DKK1 has a molecular weight of approximately 26 kDa. DKK1 features six secondary structures and contains two conserved domains rich in cysteine residues. The C-terminal Cys domain of DKK1 exhibits a fold like that of lipase, while its N-terminal domain may be associated with apoptotic signaling pathways neutralized by DKK1 antibodies. DKK1 is preferentially expressed in adult tissues, involving organs such as bone, placenta, prostate, spleen, and colon.

DKK1 is widely recognized as a primary inhibitor of the Wnt pathway and participates in the Wnt/β-catenin signaling pathway. Additionally, DKK is involved in other key signaling pathways, including the c-Jun NH2-terminal kinase (JNK) pathway, and the DKK1/CKAP4 pathway.
As an endogenous inhibitor of the Wnt signaling pathway, DKK1 plays a critical role in embryonic development and various cellular behaviors. It not only participates in processes such as cell proliferation, migration, and adhesion but also regulates cardiovascular and neural development, as well as cell apoptosis signaling pathways.

The abnormal expression of DKK1 is closely associated with various diseases.
DKK1 is expressed aberrantly in multiple cancers, including but not limited to breast cancer, prostate cancer, lung cancer, and colorectal cancer. DKK1 may induce apoptosis in tumor cells by regulating the Wnt/β-catenin signaling pathway. High levels of DKK1 expression are correlated with cancer progression and prognosis, making it a potential biomarker.

High expression of DKK1 is closely related to the occurrence and development of osteoporosis. DKK1 can inhibit osteoblast differentiation, thereby impeding bone formation. Overexpression of DKK1 leads to bone loss, negatively impacting bone health.
In Alzheimer's disease brain samples, enhanced expression of DKK1 in degenerating neurons has been observed. Furthermore, data also indicate the involvement of DKK1 in cardiovascular diseases, fibrotic diseases, type II diabetes, and other diseases. Targeted drugs and pronounced inhibitory functions make DKK1 a feasible and attractive target in many diseases.
