Fibroblast Activation Protein Alpha (FAP), also known as Integral Membrane Serine Protease or DPPIV, is a crucial member of the serine protease family. FAP is a homodimer membrane gelatinase. Under normal physiological conditions, the expression of FAP is undetectable. Under some pathological conditions, such as cancer and arthritis, FAP is selectively expressed in some cells and tissues. For example, FAP has been found to be expressed in malignant cells of soft tissue and bone. Notably, elevated FAP is generally linked to poorer clinical outcomes in diverse cancer types.

Accumulating results indicate that FAP is an important regulator of tissue repair, development, extracellular matrix degradation, and fibroblast growth in epithelial cancers. FAP is also closely linked to many cellular processes, including wound healing, tissue remodeling, fibrosis, and inflammation. Furthermore, FAP expression regulates tumor growth by participating in a variety of physiological processes, including tumor cell invasion and proliferation, angiogenesis, and immune suppression.

FAP is thought to be associated with the development of a variety of diseases, including atherosclerosis, arthritis, autoimmune diseases, and metabolic diseases. Much evidence suggests that FAP may contribute to increased disease severity.
FAP is overexpressed in many cancers, such as lung cancer, breast cancer, bladder cancer, colorectal cancer, ovarian cancer, and pancreatic cancer. Typically, FAP is abundantly expressed in the stroma. Currently, FAP has gradually emerged as a marker for cancer-associated fibroblasts.
FAP is closely associated with multiple downstream signaling pathways, including RAS/ERK, PI3K/AKT, SHH/GLI and FAK. For example, in osteosarcoma cells, FAP is involved in the regulation of angiogenesis through the ERK and AKT signaling pathways. Furthermore, in oral squamous cell carcinoma, FAP regulates cell proliferation through the ERK and PI3K/AKT signaling pathways.
