Interleukin 1 Receptor Type 1 (IL1R1) also known as CD121A, is a receptor for IL1 alpha and IL1 beta. IL-1R1 is a transmembrane protein that exists in two forms, membrane-bound and soluble, and both forms are inextricably involved in the regulation of inflammatory responses. IL-1R1 is primarily produced by immune system cells and functions throughout the body as a vital mediator in immune responses. Furthermore, IL-1R1 participates in signal transduction in the cerebral cortex. IL-1R1 also plays an important role in the cellular machinery of the central nervous system.

IL-1R1 overactivation underlies multiple human diseases, including monogenic autoinflammatory diseases, rheumatoid arthritis, diabetes, and cancer. In addition, IL-1R1 has been shown to be closely associated with a variety of central nervous system (CNS) diseases, such as amyotrophic lateral sclerosis (ALS) and multiple sclerosis. Due to its key role in the immune response, IL-1R1 has gradually emerged as the focus of drug discovery for inflammation modulation.

IL-1/IL-1R1 Signaling

IL1 is a critical factor in initiating and regulating immune responses. Two IL1 family members, IL1α and IL1β, encoded by different genes, each bind IL1R1. The IL1R1-generated complex subsequently recruits the signaling adapter proteins myeloid differentiation primary response 88 (MyD88) and IL1 receptor-associated kinase 1 (IRAK1). IRAK1 activates TNF receptor-associated factor 6 (TRAF6) and specific signaling cascades, leading to the translocation of cytoplasmic NF-κB to the nucleus and activating the transcription of inflammatory and immune genes.
