PSMA (prostate-specific membrane antigen), a type-II transmembrane glycoprotein receptor is primarily expressed in the cells of the prostate gland. PSMA is composed of three domains: intracellular, transmembrane, and extracellular. On prostate cancer cells, there is a ligand binding site in the extracellular domain where ligands bind with PSMA. PSMA is involved in the regulation of folate metabolism and facilitates the uptake of folate by prostate cells. Additionally, PSMA has enzymatic activity and participates in the cleavage of peptides.

PSMA plays a crucial role in the progression and metastasis of prostate cancer, making it an attractive target for cancer therapy. PSMA is frequently overexpressed in prostate cancer cells, roughly a hundred times higher than cells of other tissues. PSMA is the second-highest upregulated gene product in some prostate cancers, displaying an 8- to 12-fold increase over noncancerous prostate cells. According to research, higher expressing tumors in human prostate cancer are related to a faster time to progression and a higher percentage of patients experiencing relapse. Furthermore, research shows a substantial link between increased PSMA expression and prostate cancer growth.
PMSA is a membrane protein, making it an appealing target for developing monoclonal antibodies (mAbs) against it for diagnostic and therapeutic purposes. Several therapeutic anti-PSMA mAbs have been constructed, and some of these have been employed in radioimmunotherapy to specifically target cytotoxic radionucleotides to PSMA-expressing cells. Some anti-PSMA mAbs have been shown to have a therapeutic effect by increasing antibody-dependent cellular cytotoxicity (ADCC), which kills prostate cancer cells. A mAb that specifically targets PSMA, has demonstrated promising results in targeting and treating prostate cancer cells. It binds to PSMA on the surface of cancer cells, leading to antibody-mediated cell death and immune system activation against the cancer cells.
These antibodies not only directly target PSMA-positive cancer cells but can also be used as vehicles for targeted delivery of therapeutic agents, such as radioisotopes or chemotherapy drugs, to PSMA-expressing tumors, while minimizing damage to healthy tissues. A bispecific antibody that targets both the PSMA and CD3 (PMSA×CD3) can induce T-cell activation and has shown antitumor efficacy.
