Receptor tyrosine kinase-like orphan receptor 1 (ROR1) is a glycosylated type I membrane protein within the ROR subfamily of cell surface receptors, implicated in modulating neurite growth within the central nervous system. Functionally characterized as a pseudokinase devoid of catalytic activity, ROR1 potentially engages with the non-canonical Wnt signaling pathway. Although prominently expressed during early embryonic development, its presence diminishes significantly in adult tissues.

ROR1 plays essential roles in signal transduction, intercellular communication, and the regulation of cellular processes such as differentiation, metabolism, survival, and proliferation. It functions as a key modulator of Wnt signaling, with previous investigations highlighting its role as a receptor for Wnt5a. In chronic lymphocytic leukemia (CLL) cells, STAT3 induces the expression of both Wnt5a and ROR1. Additionally, IL-6 triggers STAT3 phosphorylation, leading to a time- and dose-dependent increase in ROR1 protein levels by activating ROR1 promoter activity. ROR1 maintains a favorable balance between pro-survival PI3K-AKT and pro-apoptotic p38 signaling pathways. This regulation involves ROR1 kinase-mediated activation of c-Src, as well as kinase-independent maintenance of the association between EGFR and ERBB3, resulting in ERBB3 phosphorylation and subsequent PI3K activation.

ROR1 plays crucial roles in various cancer-related processes, encompassing cell proliferation, survival, and metastasis, rendering it an enticing and promising therapeutic target. A spectrum of therapeutic modalities directed at ROR1 includes chimeric antibodies, CAR T cells, bi-specific T cell engagers (BiTEs), miRNAs, and tyrosine kinase inhibitors (TKIs). These therapeutic approaches are employed to target the ROR1 marker expressed on cancer cell lines. By discerning the most advantageous therapeutic strategies concerning ROR1 in vivo, anti-ROR1 antibodies or CAR T cells could also serve for the detection of ROR1-positive cancer cells utilizing novel technologies like biosensors. Additionally, the exploration of ROR1-targeted combination therapy with other cancer biomarkers represents a novel therapeutic avenue for cancer management.
Monoclonal antibodies conjugated with drugs and targeted towards tumor-specific antigens have enabled precise delivery of potent chemotherapy to cancerous cells while sparing healthy ones. ROR1, an oncofetal protein exhibiting minimal expression in normal adult tissues, is markedly overexpressed on the surfaces of malignant cells in mantle cell lymphoma and chronic lymphocytic leukemia. This selective upregulation renders ROR1 an appealing candidate for antibody-drug conjugate therapy, particularly in conditions like mantle cell lymphoma and acute lymphocytic leukemia, where systemic chemotherapy remains the primary treatment modality. Multiple preclinical and phase 1 clinical trials have validated the safety and efficacy of monoclonal antibody-based therapies targeting ROR1. Among these, a humanized, pioneering anti-ROR1 antibody-drug conjugate has been developed, coupling a novel anti-ROR1 antibody with a potent anthracycline derivative (PNU). These conjugate exhibits cytotoxicity against proliferating ROR1-positive malignant cells in vitro and demonstrates the ability to suppress leukemia proliferation and prolong survival in diverse murine models engrafted with human ROR1-positive leukemia.
